What you need to bring to an appointment
Disability Related Benefits

Some form of identification

Your medical summary from your GP
-
Reports from specialists that have been sent to your GP – any evidence about how your condition affects you that you didn’t include in your PIP claim form

A list of the aids or appliances you use – for example, hearing aids, walking stick
-
Copies of recent prescriptions / a list of medication you are taking

Occupational health assessments

Social care needs assessments

Education records such as a Statement of Educational Needs or Education, Health and Care plan (EHCP)

A diary of the past few weeks to assist with evidence
Universal Credit
